Dosage and Administration
Before Starting REFLUDAN Therapy1



   Determine patient's aPTT ratio: To avoid initial overdosing, do not start REFLUDAN if aPTT ratio is >2.5
   Assess renal function: Determine baseline creatinine clearance or serum creatinine
   REFLUDAN is dosed by weight up to 110 kg; patients >110 kg receive 110-kg dosage

Initial Dose



   IV bolus: 0.4 mg/kg
—  Administer bolus dose intravenously over 15 to 20 seconds
—  Immediately following administration of bolus dose, begin REFLUDAN infusion
—  CAUTION: Preparation of REFLUDAN bolus injection requires dilution following reconstitution in order to obtain the final concentration of 5mg/mL
   Continuous IV infusion: 0.15 mg/kg/hour, as long as clinically needed
—  Duration of therapy is generally 2 to 10 days
   Simple dose adjustments when initiating therapy in patients with renal impairment
